Welcome to Threshold Integrative Mental Health. Life often brings us to thresholds—moments of transition, uncertainty, loss, or new beginnings. Whether you're struggling with anxiety, depression, ADHD, trauma, grief, relationship challenges, or simply feel that life has become harder to navigate, you don't have to face it alone. I'm Asher Bar-Nes Caldwell, MSN, ANP-BC, PMHNP-BC, ACHPN, a board-certified Psychiatric Mental Health Nurse Practitioner with more than 17 years of experience as an adult and hospice nurse practitioner before transitioning into psychiatric practice. My work has taught me that every person's story is unique and that meaningful healing begins by understanding the whole person—not simply treating symptoms. I provide comprehensive psychiatric evaluations, thoughtful medication management, and psychotherapy for adolescents and adults throughout Oregon and Washington via secure telehealth, with in-person appointments available in Portland. My approach is collaborative, evidence-based, and integrative. Medication can be an important part of treatment, but it is rarely the whole story. Together, we'll explore your experiences, strengths, relationships, values, and goals to develop a treatment plan that fits you. When medication is appropriate, I strive to prescribe thoughtfully and use the least medication necessary to support lasting wellness. I have particular experience working with ADHD, anxiety disorders, depression, bipolar disorder, OCD, trauma-related conditions, grief, caregiver stress, complex medical and psychiatric presentations, and major life transitions. I also enjoy helping individuals seeking diagnostic clarification or a second opinion. Threshold Integrative Mental Health is an LGBTQ+ affirming practice committed to providing compassionate, culturally humble care to people from all backgrounds.
