Following testing, a 1-hour feedback meeting is held to review all results. You will also receive a comprehensive written report including applicable diagnoses with individualized recommendations and accommodations for school or home support. Academic Consultation: Academic testing to help identify your child's academic performance level including strengths and needs, and help inform your next steps for advocacy or further assessment. This testing cannot yield a diagnosis, but can help indicate if further diagnostic testing might be recommended. It provides you with a report of your child's skill level in specific academic areas relative to peers, and recommendations for next steps. If, after the consultation a diagnostic evaluation is indicated, your cost for the consultation would be deducted from the overall cost of the comprehensive evaluation. IEP / ETR Document Review: A review of your child's current Individualized Education Program (IEP) and Evaluation Team Report (ETR) is conducted, and I provide feedback and recommendations to clarify supports and inform advocacy. A consultation report is provided. Additional consultation for other records (504s, FBAs/BIPs, etc.) and school meeting support is also available. - Payment Options - You will receive a good faith estimate prior to the first testing session. For diagnostic assessments, 50% of the estimated service cost is due at the first scheduled testing session. The remaining balance is due at the feedback session. Cash, check, credit or debit card is accepted. A payment plan is available upon request for services over $500. a $200 deposit is required at the first appointment, with the remaining balance charged over a 6-month or 12-month pay period with no interest. A credit card authorization for automatic payments is required to be on file for payment plans. - A Note About Insurance - I do not currently accept private insurance. Unfortunately, many insurance providers do not include coverage for the assessment of dyslexia or other learning disorders. A superbill can be provided upon request, following services rendered to see if potential partial reimbursement is available directly from your insurance. Contact your carrier for information regarding Out-of-Network coverage of services. The most common billing codes used for this service are: 90791, 96136, 96137, 96130, and 96131.
