I’m Rachel Horace, a board‑certified Psychiatric Mental Health Nurse Practitioner and the founder of Core Wellness Behavioral Health. My path into mental health care began with a deep belief that people deserve to feel understood, supported, and empowered as they navigate life’s challenges. Over the years, I’ve worked in outpatient, telehealth, and community settings, helping clients move toward stability, clarity, and meaningful change. I specialize in treating anxiety, depression, mood disorders, ADHD, adjustment‑related stress, and psychotic spectrum conditions, and I take a thoughtful, trauma‑informed approach to every session. My work blends evidence‑based medication management with practical coping strategies and holistic mind‑body support, tailored to each person’s needs and pace. Clients often come to me during moments of transition, overwhelm, or uncertainty. Together, we slow things down, understand what’s getting in the way, and build a plan that feels doable and grounded.
